The inspection was a Case Management visit following an incident report regarding a resident elopement. The primary deficiency cited relates to inadequate observation protocols, specifically concerning the resident wandering off the facility without staff knowledge. Both deficiencies cited are Type B, indicating non-compliance that could become a risk if not corrected.

The inspection was conducted on 07/17/2025 as a Case Management follow-up regarding an incident report from 7/9/2025. The Licensing Program Analyst interviewed staff and reviewed resident files, noting medication changes and dementia diagnoses for the involved residents.
