This facility has concerning issues with families filing multiple complaint reports about abuse protection and care quality problems. The most recurring deficiencies involve medication management, resident safety and accident prevention, and abuse protection protocols. While all violations show provider correction dates, the repeated problems across multiple surveys from 2023-2025 suggest ongoing challenges with maintaining consistent care standards despite corrective efforts.
