Improper storage in laundry areas: opened bags of briefs, metal chairs, walkers, and clothing boxes were stored in clean linen areas; craft and activity supplies were stored in the laundry processing area, posing cross-contamination risks.
Food safety violation: multiple food items in the employee lounge, memory care kitchenette, and main kitchen were unlabeled and lacked open dates, making it impossible to determine if they were safe for consumption.
Hazardous materials/safety risk: toxic chemicals were found in an unlocked cabinet in the memory care kitchenette and adjacent dining area; knives were found unsecured in the memory care kitchenette, posing a risk to residents with impaired cognition.
