The facility underwent an unannounced Case Management visit for a Health and Safety check. The Licensing Program Analyst conducted a tour, interviewed staff, and reviewed records. No deficiencies were cited during this visit, and the facility was observed to be clean, sanitary, and in good repair.

This report details two separate complaint investigations conducted on different dates (10/20/2025 and 10/20/2025). In both instances, the allegations—improper needle disposal and failure to safeguard belongings—were found to be unsubstantiated or unfounded based on the investigation findings. Therefore, no deficiencies were cited in this report.

The investigation substantiated the allegation of unlawful eviction because the initial 30-day notice letter failed to meet legal requirements. Specifically, the notice lacked the required specific facts, including dates, places, and witnesses, and did not specify the expiration date of the notice period. Two technical violations were cited related to the formatting and content of the eviction notice.
