FACILITY EVALUATION REPORT

The inspection revealed two deficiencies, both classified as Type B. The facility was cited for inadequate staff initial training and for lacking documentation of a certified administrator. Both deficiencies require corrective action with a due date of 01/16/2026.

The inspection was conducted unannounced and covered general facility compliance. The facility was observed to have proper furnishings, sanitary bathrooms, and adequate emergency supplies. One Type B deficiency was cited regarding the failure to ensure newly hired staff complete initial training within the required timeframe.

The inspection identified critical deficiencies related to fire safety and life safety equipment. Specifically, the report cites violations for obstructed fire exits and the lack of installed carbon monoxide detectors. Immediate corrective action is required to mitigate these significant health and safety risks.

The facility underwent a Required-1 Year Inspection focusing on the infection control domain. The inspector noted that no immediate health, safety, or personal rights violations were observed during the tour of the facility. Consequently, no deficiencies were cited.

This report details multiple complaint investigations conducted on different dates (8/22/2022 and 10/25/2022). All allegations reviewed in the report, including staff throwing items, inadequate food service, and poor accommodations, were found to be Unfounded or Unsubstantiated based on the evidence gathered during the investigation.

The inspection was a required annual visit conducted by the LPA. The facility was generally observed to be clean and in good repair, with no immediate health, safety, or personal rights violations noted during the tour. Two non-compliance items were noted: the need for hand-washing posters at sinks and the submission of updated insurance/LIC500 documents.
