Type of inspection: Complaint Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: An on-site inspection related to a complaint was conducted on March 03, 2026, between approximately 12:15 PM ? 12:55 PM. A complaint was received by VDSS Division of Licensing on January 28, 2026, regarding allegations in the area(s) of: ADMISSION, RETENTION AND DISCHARGE OF RESIDENTS Number of resident records reviewed: 1 Number of interviews conducted with staff: 1 Additional Comments/Discussion: At the time of the on-site inspection, the resident identified in the complaint was no longer living there. The former resident?s record was reviewed. Specifically, the discharge summary identified the date the resident and their legal guardian were notified of the decision to discharge the resident, the reasons for the discharge, the date of the discharge, and where the resident was discharged to was reviewed. It was determined that the required notifications and justifications for the discharge were included in this summary. It was also determined that the resident was not discharged from the facility until an alternative placement had been identified: in this case, an area hospital. The evidence gathered during the investigation did not support the allegation(s) of non-compliance with standard(s) or law. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of residents present at the facility at the beginning of the inspection: 13 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of resident records reviewed: 2 Number of staff records reviewed: 2 Number of interviews conducted with residents: 2 Number of interviews conducted with staff: 1 Observations by licensing inspector: Residents were observed interacting with each other and with staff. These interactions were appropriate. The resident were noted to be appropriately dressed for the time of day, weather conditions, and the activities they were engaging in. Additional Comments/Discussion: The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected. Four (4) resident and four (4) staff files were reviewed as well as other required documentation, required postings, medication storage and medications administered, inspected the building, meals and activities, observed staff and resident interactions, etc. for compliance. The Acknowledgement of Inspection form was signed and left at the facility. The evidence gathered during this inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The applicant has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to maintain future compliance with applicable standard(s) or law. If the applicant wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Based on a review of resident records the facility failed to obtain a physical examination within 30 days preceding admission. Evidence: Resident #3 was admitted 5/12/2022 however, no physical examination was present in their file or could be verified.
A monitoring inspection was initiated on 11/08/2021 and concluded on 11/10/2021. The Administrator was contacted by telephone to initiate the inspection. The administrator reported that the current census was (14). The inspector emailed the Administrator a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed (2) resident records, (2) staff records, activities calendar, menus, fire and health reports, health care oversight, fire drills, staff schedules, etc.) submitted by the facility to ensure documentation was complete. The inspector conducted the on-site portion of the inspection on 11/10/2021. An exit interview was conducted with the Administrator on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
