An unannounced renewal inspection was completed on March 23, 2022, for Brownstone Assisted Living Facility from 3:49 p.m. to 5:34 p.m. During the entrance conference the administrator confirmed the census as twelve (12) residents in care. The sample record review contained four (4) resident records and two (2) staff records. Other areas of reviewed during the renewal study consist of the following : a medication pass observation, staff interviews, resident interviews,a building tour, staff training document , emergency food supplies, annual health inspections and annual fire inspection. An exit meeting was conducted to review the inspection findings. The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. VIOLATION: Based upon observation, the facility failed to retain at the facility all staff records. EVIDENCE: The licensee/ administrator failed to retain staff record #1 and staff record #2 in the facility / place of business.
VIOLATION: Based upon the building tour, the facility failed to maintain all furniture in a safe or good condition. EVIDENCE: During the building tour with the administrator, the licensing representative noticed that one of the night stands in room #3 had a broken dresser draw.
