The facility underwent a required Annual inspection and was generally found to be compliant in many areas, including food storage, facility cleanliness, and safety equipment checks. However, one Type B deficiency was cited regarding the failure to properly update and obtain signatures on pre-admission appraisals for two residents. The facility was given a Plan of Correction due by 10/15/2025 to address this recordkeeping issue.

This was a follow-up visit conducted to verify compliance regarding water temperature regulations. The evaluator tested multiple sinks and found all water temperatures to be within the allowable range of 105 to 120 degrees F. Consequently, the previous deficiency citation was deemed satisfied, and no new deficiencies were cited.

The inspection revealed numerous deficiencies across multiple areas, including sanitation, facility maintenance, staff training, and resident care planning. Multiple citations were issued for improper food storage, inadequate water temperatures in resident rooms, and significant structural/safety hazards like broken windows and holes in walls. The facility must address these issues promptly to ensure resident safety and compliance.

COMPLAINT INVESTIGATION REPORT

This was a follow-up inspection conducted on 09/12/2022. The primary finding relates to temperature control, where the exterior temperature was noted at 85 degrees F. The facility plans to mitigate this by purchasing and installing portable air conditioners and seeking a permanent solution.

The complaint investigation found the allegation regarding uncomfortable temperatures to be substantiated. The primary deficiency cited is the failure to maintain a comfortable temperature range for residents during extreme heat. Immediate corrective action is required to address the temperature control issues.

The inspection was a follow-up case management visit conducted to review citations from a previous annual inspection and two self-reported incident reports. The analyst noted that all staff were cleared and associated with the facility for the date of the visit. No deficiencies were cited during this inspection.
