FACILITY EVALUATION REPORT

The unannounced Case Management Visit identified a serious incident involving the use of an unauthorized restraint on a resident. Two Type A deficiencies were cited, specifically regarding the prohibition of restraints and the actual restraint of a resident's hands. The facility management took immediate action, including suspending and terminating the staff member involved, and a Plan of Correction was developed.

The facility was visited for an unannounced Case Management visit following a self-report of alleged physical abuse. The Licensing Program Analyst conducted a wellness check and found no health or safety issues. No deficiencies were cited or observed during this inspection.

COMPLAINT INVESTIGATION REPORT

This report details a complaint investigation regarding alleged mismanagement of a resident's medication. The investigation, which included staff interviews and records review, found no evidence to support the allegation. Consequently, the complaint was deemed unsubstantiated.

The facility underwent an unannounced required Annual Inspection. The inspection found the facility to be clean, sanitary, and in good repair across all areas reviewed. No deficiencies were observed or cited during this annual inspection.

The unannounced visit was conducted to open a complaint investigation and serve as a case management visit. The Licensing Program Analyst reviewed eviction procedures with the Director of Health Services. No deficiencies were cited during this visit.
