What this means for your family

While recent reviews suggest improvements under new leadership, the facility has a history of serious clinical oversights. Key Review Excerpts

My wife Lisa Kirk was transferred to #SilverOakNursingAndRehabilitationCenter in Marion Iowa at 11AM Dec. 21. She has been in excruciating pain to the point of tears since she has been there. They failed to continue her pain management plan from the University Hospital for her transition.

Long-term resident's family · 2022☆☆☆☆

My mom was there and right toward the end they wouldn’t let hospice come in. Died 5 days after that. When a resident tells you that they are going to fall get a damn wheel chair for them don’t just tell them that they won’t or no their not.

Memory care family member · 2025☆☆☆☆

The administrator is very responsive to emails and answers questions thoroughly with extra information I wouldn’t have thought to ask. I recommend this facility.

44 complaint-triggered
$29,107 in fines

This facility shows a concerning pattern of recurring complaint-triggered deficiencies, with families filing reports that led to nearly half of the 92 total violations. The most frequent problems involve resident care and treatment, safety and accident prevention, and nursing staff competency issues that persist across multiple surveys from 2022 through 2025. While all deficiencies show correction dates, the repeated nature of similar violations suggests ongoing challenges in maintaining consistent care standards.
