The facility underwent a required annual inspection conducted by the Licensing Program Analyst. The inspection covered the facility's interior and exterior areas, and the LPA reviewed resident and staff files. Overall, the report indicates that no deficiencies were identified during this inspection.

COMPLAINT INVESTIGATION REPORT

The inspection revealed several deficiencies across multiple areas. Most critically, a resident was found without necessary medication, citing an immediate health risk (Type A). Additionally, there were deficiencies noted regarding facility maintenance (broken exit door knob), staff training compliance, and missing resident reappraisals (Type B).
