What this means for your family

Silver Creek offers a strong rehabilitation program and a generally friendly staff, making it a viable option for many. However, given the serious reports of patient elopement and falls, we strongly advise families to ask the administration specifically about their security protocols for dementia patients and their fall-prevention monitoring systems before committing.

Silver Creek Rehab and Healthcare Center receives high praise for its attentive nursing staff and effective physical therapy programs, with many families noting that their loved ones feel well-cared for and safe. However, there are serious, recurring concerns regarding patient safety, specifically reports of residents wandering off the premises, falls resulting in significant injuries, and the loss of personal belongings. While the facility is frequently described as clean and friendly, potential families should be aware of these critical lapses in supervision and security.

Questions for Your Tour

  • 1Given that physical and occupational therapy are noted strengths here, how do you integrate those services into a resident's daily routine to ensure they stay active?
  • 2With the current staffing levels, what specific protocols do you have in place to ensure residents receive timely assistance during mealtimes and that food is served at the proper temperature?
  • 3I noticed some concerns regarding resident safety and wandering; could you walk me through your monitoring systems and how you prevent falls or unauthorized exits?
  • 4How do you manage the security of personal belongings in resident rooms to ensure items are tracked and protected?
  • 5Since the memory care program is a newer focus for the facility, what specialized training does your staff receive to support residents with cognitive needs?
  • 6How do you handle medical emergencies or urgent changes in a resident's condition during evening or weekend hours when staffing levels may be different?

Key Review Excerpts

My aunt was only there three days and fell MULTIPLE times and was left on the floor for HOURS her very first night.

Memory care family member · 2024☆☆☆☆

Their doors are unlocked and open from 8am-8pm and although they had quite a lot of patients with dementia they did not monitor patients well enough. My dad walked out of the facility before 8pm and was found by Bristol police around 9 am on busy Hope St.

Long-term resident's family · 2022☆☆☆☆

My dad just arrived at Silver Creek this week. Case worker Fran is great and all over everything I needed to get dad settled. I had a check in with PT and OT and today I was able to observe OT help my dad work on his eating skills.

Families have filed multiple safety and abuse concerns with federal inspectors, including recent accident hazards and resident protection failures. The facility shows recurring problems with wound care and pressure ulcer prevention, accident prevention, and medication management across multiple years. While all deficiencies have correction dates, the pattern of repeated issues in critical safety areas, combined with ongoing family complaints, suggests persistent quality concerns that warrant careful consideration.
