What this means for your family

While many families report excellent, compassionate care, there is a clear pattern of inconsistent service, particularly on weekends and regarding nutritional support. When touring, we strongly recommend asking for the specific weekend staffing ratios and how the facility monitors patient weight and intake during short-term rehabilitation stays.

Questions for Your Tour

  • 1I noticed that staffing levels can sometimes feel different on weekends; could you walk me through how you ensure consistent care and supervision for residents during those off-hours?

  • 3With your 3-star CMS staffing rating, what steps is the facility taking to ensure that nursing staff have enough time to provide personalized attention to each of the 50 residents?

  • 5Could you describe the daily activity schedule and how you encourage residents to participate in social or therapeutic programs?

Key Review Excerpts

My 85 year old Dad went here for a week following heart surgery. When he arrived here for the first 24 hours no one came in to check on him no one let him know when meals were being served and no one bought him any meals.

Long-term resident's family · 2023☆☆☆☆

On the flip side, the food was awful for my mom at Progressive (I have pictures) AND within my mom’s 7 day stay- she lost a total of 10 LBS. My mom went into rehab to gain strength and she actually left Progressive WAY weaker than when she got discharged from the hospital.

Short-term rehab family member · 2025☆☆☆☆

My dad was placed at Progressive October 2024. From the first day, I knew he was in a great place, despite him not really wanting to be there. Any issue or concern I had, was addressed immediately and taken care of.

Families have filed multiple complaints against this facility, resulting in findings related to accident prevention, staff training, and abuse prevention policies. The most recurring issues involve fire safety and building code violations, medication management problems, and quality of care concerns including wound care and respiratory safety. While the facility has provided correction dates for most deficiencies, the pattern of repeated violations across safety, staffing, and care areas suggests ongoing operational challenges that families should carefully consider.
