What this means for your family

While Meadow View is praised for its small, intimate setting and effective rehab team, the reports of inconsistent hygiene and night-shift care are serious red flags. When touring, we strongly recommend asking for a detailed plan on how the facility ensures residents are assisted with meals and hygiene during off-hours, and specifically request to speak with staff from the night shift.

Meadow View Health Services receives polarized feedback, with some families praising the small, home-like atmosphere and compassionate care, while others report serious concerns regarding neglect and hygiene. While several reviewers highlight the kindness of the staff and the effectiveness of the rehab team, multiple reports of residents being left soiled or unassisted with meals suggest significant inconsistencies in daily care standards.

Key Review Excerpts

As a healthcare profession who works in long term care, I have to say that ALL the staff do a great job. From the ED, nurses, CNAs, housekeeping, dietary, social worker, and activities, they all were top notch.

Memory care family member · 2024★★★★★

Finding our dear loved one repeatedly soiled for days without changing, leaving her meals across the room when she can't get to them, and her medication not given was disgraceful.

Long-term resident's family · 2018☆☆☆☆

There were morning staff that were very kind and understanding of my underlying conditions as well as my disability. The social worker there is phenomenal. He really does his job. Well, there are some stuff that are not very nice and not understanding especially some of the night staff.

3 complaint-triggered

Families have filed complaints that triggered inspections, raising concerns about safety supervision and care planning practices. Meadow View has recurring issues with resident rights notifications, accident prevention, and fire safety systems that persist across multiple surveys from 2022-2024. While all deficiencies show correction dates, the pattern of repeated violations in similar areas suggests ongoing compliance challenges families should investigate during visits.
