“On February 19, 2023, at approx 8pm, my mother fell in her bathroom at Serentiy Care Mid Valley. She was not discovered by staff until 7am the next morning. Residents are supposed to be checked every two hours, and my mother was supposed to be checked hourly due to fall risk.”
