What this means for your family

This facility is highly regarded for its activities program and friendly staff, making it a warm environment for many. However, because multiple reviewers have raised serious concerns about clinical oversight and weekend staffing, we strongly recommend that you visit on a weekend and ask specific questions about how medical emergencies and daily care needs are managed during off-hours.

Questions for Your Tour

  • 1With the recent changes in management, what specific improvements have been made to the facility's physical environment and room maintenance?
  • 2Given that staffing can sometimes feel different on weekends, how do you ensure consistent quality of care and supervision for residents during those shifts?
  • 3What protocols do you have in place to ensure accuracy and safety in medication management, especially given the complexity of clinical care for residents?
  • 4I noticed the facility offers a wide variety of activities; could you walk me through a typical week of programming and how you encourage residents to participate?
  • 5How does your nursing team communicate with families when there is a change in a resident's medical status or if a clinical concern arises?
  • 6Since you have a 5-star CMS rating, what do you believe is the biggest strength of your current care team in maintaining that standard of excellence?

Key Review Excerpts

It is an older facilities so you’re not gonna have the glamour, but the beds are comfortable. The food is great. He eats every bite of it And the nurses and the CNA‘s and the therapy people are like family.

Long-term resident's family · 2025★★★★★

This place is barely held together by management during the week days, but it completely falls apart on the weekends. Getting out of bed is a natural human desire as well as a necessary health requirement and preventative health measure.

Family member · 2024☆☆☆☆

My sister had been in their rehab once before having suffered a brain bleed because a doctor did not prescribe a blood pressure medication for her. She had a stroke and then other issues with her right hip. This last stay I got a call from a nurse telling me she thought my sister was having a stroke.

Both RN and total nursing hours are below national benchmarks. This can mean less clinical attention per resident, so ask about their staffing plan.

1 complaint-triggered

This facility has a mixed record with 11 deficiencies across four surveys, including two complaint-triggered issues where families filed reports about medication management and safety equipment. The most recurring problems involve care planning (appearing multiple times), medication management, and nursing competencies. While all deficiencies show correction dates, the pattern of repeated care planning issues and recent 2025 violations in core areas like resident rights and nursing competencies suggest ongoing operational challenges families should investigate during visits.
