While recent reviews are overwhelmingly positive regarding the facility's cleanliness and community engagement, the serious concerns regarding hygiene and staff responsiveness in 2023 cannot be ignored. When touring, we strongly recommend asking for a detailed explanation of their current protocols for room cleaning and how they ensure timely responses to call lights, especially during evening and weekend shifts.

Salem Springlake Health & Rehabilitation Center shows recurring issues across fire safety, electrical systems, and emergency preparedness that persist across multiple surveys, with the same fire safety and electrical problems reappearing in 2023 and 2025. The facility also had deficiencies in care planning and infection control in earlier surveys. While all violations have correction dates, the pattern of repeated safety issues in recent years suggests ongoing challenges with maintaining compliance standards.
