While the facility offers strong life enrichment programs and some dedicated staff members, the recurring reports of hygiene issues and neglect for bedridden residents are significant red flags. If you choose this facility, you must maintain a daily, in-person presence to ensure your loved one's basic needs are met and to advocate for their care.

Key Review Excerpts

Of countless visitations I’ve made to Nursing Homes & Rehab facilities, SADIE G. MAYS ranks HIGHEST in its LIFE ENRICHMENT Program, led by Director, Ms. Stromboli D. Curier.

Visitor · 2024★★★★

My grandfather spent 4 days at Sadie G. Mays. On his 4th day there I visited him in response to a call home from him saying he was not getting the care he needed. When I arrived there had not yet been an attempt to completely sit him up in his bed.

Grandchild of resident · 2024☆☆☆☆

My mother was there for the time Medicare paid and she did no rehabilitation. They discharged her without her oxygen and medication on a Friday afternoon, asking us to pick up her medication the next day.

This facility has concerning patterns with families repeatedly filing complaints that triggered 31 of 49 total deficiencies across multiple surveys. The most recurring problems involve medication management errors, resident rights violations including failure to properly notify families of incidents, and infection control failures. While all deficiencies show correction dates, serious issues persist across time periods, including recent complaints in 2025 about abuse reporting failures and inadequate care planning, suggesting ongoing systemic problems that families should carefully investigate before placement.
