What this means for your family

While some families report excellent experiences with the rehabilitation team and facility cleanliness, the recurring reports of medication errors and neglect are extremely concerning. If you consider this facility, we strongly recommend conducting unannounced visits and asking for a detailed plan on how they ensure medication safety and adequate staffing levels during off-hours.

Key Review Excerpts

Upon the first week, a nurse came and administered her meds into her PICC line. After the nurse walked out, my mother in law looked down to see that it was not her name on the meds. They gave her someone else’s meds!!!

Long-term resident's family · 2025☆☆☆☆

In 8 weeks my husband was able to heal and gain enough strength while in Royalton Rehabilitation wing to be able to come home to continue his therapy and healing in a familiar environment. There are many caring and competent individuals at this facility and their therapy department is fantastic.

Rehab patient's spouse · 2025★★★★

I work for an assisted living facility in New Jersey and have high expectations for situations such like this, and I cannot thank each and every care giver/nurse enough for their kindness, ease to communicate with, and overall professionalism.

This facility has significant ongoing problems with families filing multiple complaint reports about care quality, staffing competency, and medication management. The facility shows persistent issues across surveys with fire safety systems, nursing staff training, and infection control that continue appearing year after year. While all deficiencies show correction dates, the recurring pattern of similar problems—particularly the complaint-triggered issues around resident safety and care standards—suggests systemic challenges that may impact resident well-being.
