The following deficiency was found during the on-site compliance inspection conducted on May 30, 2024:

Based on observation, documentation review, and interview, the manager failed to ensure toxic materials stored by the facility were stored in a locked area and inaccessible to residents. The deficient practice posed a risk to the physical health and safety of a resident. Findings include: 1. The Compliance Officers observed the following materials in an unlocked cabinet in the laundry closet. The list provided is a sample of the chemicals that were found in the cabinet: -A spray bottle of Spic and Span Antibacterial Spray Cleaner -A canister of Lysol -A spray bottle of Fantastik All Purpose Cleaner with Bleach -A spray bottle of Oxygen Power All Purpose Spot Remover -A bottle of Clorox Toilet Bowl Cleaner -A spray bottle of Scrub Free Total Bathroom Cleaner 2. A review of the facility's policy and procedures reviewed by E1 April 2024 revealed a policy titled, "Environmental and Physical Plant Safety," which stated in section 15, "Poisonous and toxic materials will be in labeled containers and stored in a locked area separate from food preparation and food storage areas, dining areas and medications and are inaccessible to residents." 3. In an interview, E2 acknowledged toxic materials stored by the facility were not stored in a locked area.
