What this means for your family

Key Review Excerpts

The rehab staff went above and beyond and we are so grateful for them. I would highly recommend Riverwood for Rehabilitation!

Rehab patient's family · 2022★★★★

My father's stay at this facility for PT/OT was a nightmare. Despite his severe knee pain and swelling, which rendered him unable to undergo therapy, his condition was left untreated. This neglect led to the development of bedsores and a large rash covering most of his back.

Rehab patient's family · 2024☆☆☆☆

The food is fantastic the people are happy to get whatever you need even in the middle of the night or day ive had a lot of pain and have been a little grouchy at times please forgive me this place i would bring my mom to.

Families have filed reports triggering inspections, with recurring issues in medication management, resident rights, and safety protocols. The facility shows persistent problems across multiple surveys, including medication errors, privacy violations, accident hazards, and emergency preparedness gaps. While all deficiencies report correction dates, the pattern of repeated violations in core care areas suggests ongoing quality concerns families should carefully evaluate.
