The facility failed to ensure that a resident's completed care plan was signed by a physician or physician extender within 15 days of assessment. Specifically, a review of Resident #1's care plan dated 12/30/25 showed it lacked the required signature from the Primary Care Provider.

The facility failed to notify the Division of Health Service Regulation that the evacuation capabilities of certain residents had changed. Specifically, for 2 of 5 sampled residents, physical and cognitive impairments were present that could prevent independent evacuation, which differs from the facility's license for 6 ambulatory residents.

The facility failed to ensure quarterly pharmaceutical reviews were completed for 2 of 3 sampled residents. Specifically, Resident #1 had no pharmacy reviews available after March 2021, and the Administrator admitted to overlooking the pharmacy review service due to other facility responsibilities.

The facility failed to ensure quarterly pharmaceutical reviews were completed for 2 of 3 sampled residents. The Administrator failed to request medication reviews from the contracted pharmacy, resulting in a lack of documented medication monitoring for Resident #1 and Resident #3.

The facility failed to clarify medication orders for one resident. Specifically, the resident's FL2 form dated 1/05/17 omitted an order for mirtazapine, even though the pharmacy continued to dispense the medication based on an older order and the resident's MAR showed the medication was being administered.
