Type of inspection: Renewal Date of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: September 10, 2025, from 9:35 a.m. until 12:24 p.m. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of residents present at the facility at the beginning of the inspection: 17 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of resident records reviewed: 2 Number of staff records reviewed: 2 Number of interviews conducted with residents: 3 Number of interviews conducted with staff: 2 Observations by licensing inspector: The Licensing Inspector toured the community and observed the residents during activities and meals. The Licensing Inspector reviewed the following at the time of inspection: sample of resident and employee records, medication administration, fire drills, emergency drills, pharmacy review, menus, activity calendars, verified appropriate amount of liability insurance, and dietician report. Additional Comments/Discussion: None An exit meeting will be conducted to review the inspection findings. The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected. Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area. Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice. Based on observation and staff interview, the facility failed to store cleaning supplies in a locked area. Evidence: 1. During a tour of the facility on 9/10/2025, licensing inspectors observed Clorox Clean-Up Disinfectant Cleaner with Bleach in the unlocked men?s spa area. 2. During a tour of the facility on 9/10/2025, licensing inspectors observed cleaning supplies, including Clorox Clean-Up Disinfectant Cleaner with Bleach, Clorox Clinging Bleach Gel, and Lysol Bleach, in an unlocked public restroom. 3. Staff 2 acknowledged the cleaning supplies were left out, stating ?the bathrooms are cleaned frequently?. 4. Photo evidence taken.

Based on resident record review and staff interview, the facility failed to prepare and provide a statement to the prospective resident and his legal representative, if any, that discloses information about the facility. Evidence: 1. Resident 1?s and Resident 2?s negotiated Disclosure Statement on file did not include whether or not the facility has an on-site emergency electrical power source for the provision of electricity during an interruption of the normal electric power supply. 2. Staff 3 stated the facility had a Disclosure Statement that was updated to include the on-site emergency electrical power source, but they had not started using this version.

Number of residents present at the facility at the beginning of the inspection: 17 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. The facility was clean and odor free. The facility was decorated for the summer. Water temperatures were all within range and are monitored regularly. Postings were as required, and activities and menus followed. Number of resident records reviewed: 4 Number of staff records reviewed: 3 Number of interviews conducted with residents: 4 plus 2 family members. Number of interviews conducted with staff: 2 Observations by licensing inspector: Four resident and three staff records were reviewed and found to be following the standard requirements. Interviews with staff, residents and two family members noted complete satisfaction with service with a family member noting staff go beyond what they could have expected in taking care of their family member, Additional Comments/Discussion: Fire ? 8/1/23 Health ? 8/9/23 Outside inspections and reviews were completed and follow-up as applicable. All drills were as required. An exit meeting was conducted to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. A monitoring inspection was initiated on 9/2/21 and concluded on 9/10/21. The administrator was contacted by email to initiate the inspection. The administrator reported that the current census was 15. The inspector emailed the administrator a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed 3 resident records, 2 staff records plus additional background checks for 3 staff, emergency drills, outside inspections, staff schedules, menus and activity schedules submitted by the facility to ensure documentation was complete. The inspector conducted the on-site portion of the inspection on 9/10/21. An exit interview was conducted with the Administrator on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
