FACILITY EVALUATION REPORT

The facility underwent an Annual Inspection and the Licensing Program Analyst noted several positive observations regarding facility safety, resident care, and recordkeeping. No deficiencies were cited during the inspection visit.

The inspection identified several deficiencies across multiple areas, including medication storage, staff record keeping, and resident care planning. Most notably, the finding of accessible medications on the kitchen counter constitutes a Type A immediate health risk. Other deficiencies relate to incomplete staff health documentation and missing annual medical assessments for a resident with dementia.

The inspection revealed multiple deficiencies across several critical areas, including immediate health risks related to water temperature and unsecured hazardous materials. There were also several non-compliance issues concerning staff documentation (TB tests, first aid), medication management, and required resident assessments/reappraisals. The facility must address these deficiencies promptly to ensure resident safety and regulatory compliance.

The physical facility was generally found to be clean, and basic safety measures like hand sanitizing and PPE availability were noted. However, the report identified deficiencies related to outdated or missing administrative documentation and the administrator's certificate status.
