This report details an investigation into complaints received regarding resident care. Although allegations were made concerning lack of medical assistance and unclean living quarters, the investigation found insufficient evidence to substantiate any violations. Therefore, the allegations were deemed unsubstantiated.

This report details an unannounced complaint investigation conducted on 10/23/2025. The department investigated allegations regarding staff locking a resident's bathroom door and issues with medication refills. Ultimately, the allegations were deemed unsubstantiated due to insufficient evidence.

The case management visit identified multiple critical deficiencies across several areas of care. Key issues include unsecured oxygen tanks, lack of resident call systems, and failure to maintain required criminal background clearances for staff. All cited deficiencies were classified as Type A, indicating a direct and immediate risk to the residents' health and safety.

The facility underwent an unannounced Case Management visit on 09/12/25. The inspector observed general areas, including resident rooms, common areas, and the kitchen, noting that basic services like running water and food supplies were adequate. No deficiencies were cited during this visit.
