The facility failed to ensure that one of five sampled residents had completed required tuberculosis (TB) testing upon admission. Specifically, Resident #3 only had a negative chest x-ray on file rather than the required negative TB skin or blood test. Additionally, facility management and the Resident Care Coordinator demonstrated a lack of knowledge regarding the proper protocols for TB testing upon admission.

Staff failed to consistently assist Resident 64 with showers and sponge baths. Interviews with medication aides and PCAs revealed that when residents refused showers, staff often failed to attempt assistance on different shifts or document the refusals. Additionally, the resident's hair was only washed when they were assisted with showers, and staff only notified the Director of ongoing refusals.

The facility failed to provide personal care according to the resident's care plan for one resident, as evidenced by oily, unwashed hair and a lack of documentation for a sponge bath or shower for 10 out of 14 days. Additionally, there was no completed care plan on file for the resident as of the inspection date.

The facility failed to maintain a matching therapeutic diet menu for all physician-ordered therapeutic diets to guide food service staff. Specifically, there was no menu available for mechanical soft (MS) diets with chopped meats, making it impossible to determine if residents receiving these diets were being served appropriate meals.
