What this means for your family

This facility has a dedicated core of staff who receive high praise, particularly in physical therapy and activities. However, families should be aware of significant, recurring complaints regarding communication and administrative follow-through. We strongly recommend visiting in person to assess the facility's current cleanliness and asking for a direct point of contact for medical updates to mitigate the communication issues noted by others.

Key Review Excerpts

The Lafayette facility is absolutely the best facility I have ever walked into where it didn’t smell like a nursing facility or hospital. I was a CNA for over 15+ years and I know all about nursing homes.

Former CNA · 2025★★★★★

Third shift improved drastically after we requested a different nurse to care for my mother. She had a great night the next night. My husband called the administrator Craig Perez about the issue. In the midst of a crisis in his own family I believe he handled ours professionally and compassionately.

Long-term resident's family · 2024★★★★

After two weeks of being in the facility, she had three wounds with one needing stitches and a trip to the hospital. We had to repeatedly ask for testing to be done. Only after the third time did they check her out and test her.

Clean record — no deficiencies

This facility shows concerning patterns with food safety problems persisting across all three surveys and families filing complaints about resident protection and care planning. The most recurring issues involve food handling and storage, care planning, and quality of life concerns including pressure ulcer prevention and respiratory care. While the facility reports correcting all deficiencies, the repeated food safety violations and five complaint-triggered deficiencies suggest ongoing care quality concerns that warrant careful evaluation during any visit.
