What this means for your family

This facility receives high praise for its cleanliness and administrative leadership, but families should be cautious due to serious, recurring reports of patient falls and communication gaps. We recommend conducting an in-person tour to observe staff-to-resident ratios and asking specifically about their fall prevention protocols and how they handle discharge planning for rehab patients.

Google Reviews

Google Reviews

66 reviews on Google
PruittHealth - Fitzgerald receives highly polarized feedback, with a large volume of glowing 5-star reviews praising the facility's cleanliness, friendly staff, and the leadership of the administrator. Conversely, several families report critical failures in patient safety, including multiple falls, poor communication, and inadequate rehab outcomes. Prospective families should be aware of these starkly different experiences, as some report exceptional care while others describe serious neglect.

Key Review Excerpts

The building has recently been updated and it is very clean. The staff is friendly and appear to enjoy their job. The front office s always ready to listen and answer any questions.

Long-term resident's family · 2023★★★★★

My dad was recently there for rehab & the staff was not present. The administration staff such as the admit person, Socual Services, all work remotely. The Social worker dropped the ball when setting up home health for my father so when he was discharged he didn’t have any help at home.

Rehab patient's family · 2026☆☆☆☆

Don’t even consider if you love your love ones. They will call you with good intentions that aren’t considered or even respected once the person is there and then every safety measure is a restraint. From Tuesday-Friday, my loved one has fell numerous of times and is now awaiting on a ventilator in Jacksonville for me to arrive tomorrow just to say my goodbyes.

PruittHealth-Fitzgerald shows a pattern of recurring building safety and resident assessment issues across all three federal surveys, with problems in fire safety systems, electrical equipment, and care planning appearing multiple times. While all 16 deficiencies have correction dates, the persistence of similar issues suggests ongoing operational challenges that families should discuss during visits to ensure current compliance.
