The facility underwent an Annual 1-year required inspection on August 27, 2025. The Licensing Program Analyst conducted a thorough tour and reviewed required documentation. No deficiencies were cited during this visit, indicating full compliance with current licensing standards.

The unannounced Annual Required inspection revealed several areas needing attention, primarily concerning resident care planning and participation. Specifically, the facility was cited for not meeting requirements regarding resident participation in decision-making and for deficiencies found in a sample review of Needs & Service Plans. The facility was given a Plan of Correction due by 08/20/2024 to address these issues.

The unannounced annual inspection conducted on August 11, 2022, found the facility to be clean and tidy with no immediate safety hazards observed. The LPA reviewed various operational areas, including infection control, medication storage, and general safety protocols. No deficiencies were cited during this inspection.

The unannounced annual inspection observed several positive aspects, including adequate infection control practices, sufficient supplies, and appropriate storage of medications. However, three minor deficiencies were noted: the kitchen needs a trash receptacle with a foot-operated lid, a hand washing sign should be placed by the kitchen sink, and the facility must submit an updated Emergency Disaster Plan by a specified date.
