Key Review Excerpts

My mother’s light had been on over 15 minutes , but that was the conversation of entertainment.

Memory care family member · 2025☆☆☆☆

They didn't report to hospice when she fell. Then they claimed they did. So how could they when the administrator didn't know about the fall. Which resulted in a brain bleed for 5 days.

Long-term resident's family · 2023☆☆☆☆

The owner and herself have been working to improve Providence as it used to be. So my sisters and I placed my Mom in this facility. I can honestly say they took very good care of my Mom.

6 complaint-triggered

Providence Healthcare has a concerning pattern with 17 deficiencies across three surveys, including two complaint-triggered issues where families filed reports about infection control and environmental safety. The facility shows recurring problems with infection control (appearing in multiple surveys), resident rights, and care quality. While all deficiencies were reportedly corrected, the pattern of repeat violations in key areas like infection prevention suggests ongoing challenges with maintaining consistent standards.
