What this means for your family

While the facility receives praise for its therapy and professional coordination, the recent reports of delayed emergency responses and severe medical neglect are critical red flags. If you are considering this facility, we strongly recommend requesting their most recent state survey results and asking specifically about their protocols for patient monitoring and emergency escalation.

Porthaven Post Acute presents a stark contrast between highly positive reviews from professional partners and recent, severe allegations from family members regarding patient safety. While many reviewers praise the new management and staff attentiveness, multiple families have reported critical incidents involving delayed emergency response, severe pressure ulcers, and allegations of neglect. Families should exercise extreme caution and conduct thorough due diligence given the serious nature of the negative reports.

Key Review Excerpts

My mother was admitted and on her first night, she called for assistance to use the restroom—no one came. She fell, fracturing her sternum and three ribs, and the facility waited eight hours before calling an ambulance.

Families have repeatedly filed complaints that triggered 34 of 57 total deficiencies, raising serious concerns about resident protection from abuse and neglect, nursing staffing adequacy, and safety hazards. The facility has faced multiple abuse/neglect violations across several years, including a severe incident in 2024, alongside recurring issues with insufficient nursing staff and accident prevention. While all deficiencies show correction dates, the persistent pattern of family complaints suggests ongoing care quality problems that warrant careful consideration.
