Questions for Your Tour

  • 1Given your facility's strong reputation for physical and occupational therapy, how do you ensure that the nursing team stays closely aligned with the therapists to monitor a resident's clinical progress?
  • 2I noticed that communication is a top priority for families; could you walk me through the specific process for how family members are updated when there is a change in a resident's medical status?
  • 3How does your team manage medication administration to ensure accuracy, especially during shift changes or busy times of the day?
  • 4In the event of a sudden change in health or a clinical complication, what is the protocol for escalating care and ensuring the attending physician is immediately involved?
  • 5Since you have a 120-resident capacity, what steps do you take to ensure that nurse response times remain prompt and consistent throughout the day and night?
  • 6I see you actively engage with feedback online; how do you use that input from families to improve the daily experience and quality of life for your residents?

Key Review Excerpts

I was referred here for therapy after trialing outpatient at one of the 'big' therapy practices. I didn't want to be treated like a number or have a tech/assistant barking exercises at me. Here at SRI, I was treated with respect and courtesy, and my treatment plan was handled by an amazing Physical Therapist and team.

Outpatient therapy patient · 2025★★★★★

The only negative thing I can say is the younger staff needs to learn how to knock on the door when entering a patient’s room. These people have already been through enough with loosing their independence, showing them some dignity is important.

Family member of patient · 2025★★★★★

Almost killed my father and he is in the ER with pneumonia and a UTi which we told them three times to check- worst nurses ever , especially on the weekend! do not go here!!
