What this means for your family

This facility's physical therapy department is highly regarded and may be an excellent choice for short-term rehabilitation. However, families should be extremely cautious regarding medication management and meal quality; we recommend conducting unannounced visits during evening or weekend hours to observe staffing responsiveness firsthand.

Patriot Health and Rehabilitation Center receives highly polarized feedback, with reviewers frequently praising the therapy department and facility cleanliness while expressing significant concerns regarding nursing responsiveness and meal quality. While some families report excellent, compassionate care for long-term and memory care residents, others describe critical failures in medication management and basic patient needs. Prospective families should be aware of the inconsistency in care standards and the recurring complaints about staffing levels.

Key Review Excerpts

The physical therapy group was amazing they get a 5 star but the staff was another story i was supposed to get a shot for blood clots every night one night nurse never gave them to me at all.

Rehab patient · 2025☆☆☆☆

My mother was sent to the facility for in house rehab. I’m shocked at the way she was treated. There were several times that she never got meals and when she did the food was so cold and hard it appeared that it had been sitting out.

Long-term resident's family · 2024☆☆☆☆

Patriot also has a 16 bed memory unit for those loved ones that are dealing with dementia . The therapy and nursing staff are top notch and works with the patients to achieve their optimal goals.

Families filed complaints that triggered recent inspections in 2025 and 2023, revealing ongoing issues with care quality, nursing competency, and dietary services. The facility shows recurring problems across multiple surveys with pressure ulcer care, accident prevention, nursing staff competency, and food service quality. While all deficiencies have correction dates, the pattern of repeated violations in core care areas suggests persistent operational challenges that families should carefully evaluate.
