What this means for your family

While the rehabilitation therapy team is frequently cited as a strength, the recurring, severe reports of neglect and unsanitary conditions are deeply concerning. If you are considering this facility, we strongly recommend conducting an unannounced visit during a weekend or evening shift to observe staffing levels and hygiene standards firsthand.

Google Reviews

Google Reviews

Ontario Center for Rehabilitation and Healthcare receives highly polarized feedback, with a significant number of families reporting severe neglect, unsanitary conditions, and chronic understaffing. While some reviewers praise the rehabilitation therapy team and specific staff members, the recurring reports of residents left in soiled bedding, dehydration, and lack of responsiveness to basic needs present a serious cause for concern. Families considering this facility should be aware of the stark contrast between the positive experiences of some and the distressing accounts of others.

Key Review Excerpts

My father 89 was admitted to this place for rehab, less than 24 hrs . I found him covered in his own feces. The bathroom was also covered.

Rehab patient's family · 2024☆☆☆☆

The actual therapists were good but the care was awful. They didn’t have rails on my moms bed and she slid out of it. They wouldn’t come and take her to the bathroom and she would be humiliated bc she would eventually just have to go in her bed.

Long-term resident's family · 2023☆☆☆☆

The activity department is fantastic! They really brighten the residents' days. It's a wonderful sight to see.

Families have filed multiple complaints triggering 10 deficiency investigations, with serious concerns including a recent abuse/neglect violation and nursing staffing shortages. The facility shows persistent problems with daily care assistance, resident rights protection, and medication management across multiple surveys from 2021-2025. While all deficiencies show correction dates, the recurring nature of issues, especially recent high-severity violations around resident safety and adequate staffing, suggests ongoing quality challenges families should carefully evaluate.
