What this means for your family

While the admissions team is frequently cited as helpful and compassionate, there is a recurring pattern of serious medical and safety concerns reported by families. We strongly recommend that you conduct an unannounced visit and specifically ask for the facility's recent state survey results and their protocols for medication management and fall prevention.

52 reviews analyzed
Oakview Nursing & Rehabilitation Center receives polarized feedback, with many reviewers praising the admissions and support staff, particularly Ashleigh Thomason, for their personal attention and kindness. However, several families have raised serious, critical concerns regarding the quality of medical care, including allegations of medication mismanagement, neglect, and physical injuries occurring during short-term stays. Families considering this facility should weigh the reported warm, homelike environment against these significant medical safety concerns.

Clean record — no deficiencies

Families filed complaints that triggered inspections revealing problems with care planning and treatment quality, though these issues were corrected. The facility shows recurring deficiencies in infection control across both surveys, along with problems in care planning, quality of life support, and resident rights. While all violations have been addressed with correction dates, the pattern of repeated infection control issues and the complaint-triggered deficiencies warrant careful consideration during your visit.
