Key Review Excerpts

Missed meds and doses. No response to med change requests for almost a week. Scheduled 1 staff to pull meds that they require 2 trained staff for resulting in waiting hours extra for treatment.

Family member · 2025☆☆☆☆

They are "short-staffed" on the weekends so he was left in soiled diapers (he can communicate but needs assistance walking, he can tell you if he needs to use the restroom) which led to a UTI and a fever Oaklawn staff didn't even know he had as they weren't checking on him!

Long-term resident's family · 2020☆☆☆☆

Very good friendly and professional care. Doing a great job helping my mother recover from a fall and hip fracture.

Oaklawn Care & Rehabilitation Center has a concerning pattern of deficiencies with families filing complaints about safety hazards, abuse reporting, and care planning issues. The facility shows recurring problems with safety supervision, abuse/neglect protocols, and basic care quality across multiple surveys from 2023-2025. While all violations appear to have correction dates, the repeated nature of safety and care issues, combined with six complaint-triggered deficiencies, suggests ongoing challenges families should investigate thoroughly.
