Key Review Excerpts

My mom spent the last 40 days of her life at Oak Hill Rehab and they were an absolute nightmare. It’s impossible to run a successful facility when there is only 1 nurse and 2 CNA’a who care, and when a critical concern is brought to the head of nursing, it’s disregarded and swept under the carpet.

Long-term resident's family · 2026☆☆☆☆

My sister was at Oak Hill Rehab from a Friday evening thru the following Saturday morning... Her wound went without being redressed for 4 days so instead of only losing two toes she lost her forefoot. Please do not trust this facility.

Family member · 2025☆☆☆☆

My father was sent here after having a stroke and the care he received was FANTASTIC!! I’m a nurse who worked many years in long term care and rehab and know how most of these places give terrible care but Oak Hill Health and Rehab has top notch nurses, CNA’s, PT and every other position.

Oak Hill Health & Rehabilitation has shown persistent issues with medication management appearing across multiple surveys from 2022 to 2024, along with recurring problems in care delivery and food service. Families have filed complaints leading to findings about medication labeling, nutrition deficiencies, and failure to report suspected abuse. While the facility corrects deficiencies when cited, the pattern of repeated violations in critical care areas suggests ongoing challenges with consistent quality oversight.
