What this means for your family

This facility has a highly regarded rehabilitation team that has helped many patients recover quickly, making it a strong contender for short-term therapy. However, families should be vigilant regarding communication and basic care; we strongly recommend visiting frequently and asking for a direct contact person for updates, as multiple reviewers noted difficulty reaching staff.

Evergreen Woods receives highly polarized feedback, with many families praising the rehabilitation therapy team for helping loved ones regain mobility after surgeries. However, there is a recurring pattern of serious allegations regarding neglect, poor communication, and understaffing, particularly during weekends and off-hours. Families considering this facility should weigh the strong clinical outcomes reported by some against the significant concerns regarding basic hygiene and responsiveness.

Key Review Excerpts

The therapy team was awesome as well ! I would like to thank All the staff @ Evergreen Woods for everything they did for my mom. I highly recommend Evergreen Woods for Rehabilitation and or Long Term Care arrangements !!

Long-term resident's family · 2025★★★★★

My father-in-law was neglected severely in the facility resulting in sepsis, bedsores, severe UTI because of poor catheter care, horrible personal care and losing over 15 lbs from only being in this facility 2.5 weeks!

Family member · 2016☆☆☆☆

The physical therapy staff was exceptional. Especially my assigned therapist Sherry. She got me walking only days after a total hip replacement. However with today's technology there is no reason to provide food consistently that is colder than room temperature.

Rehab patient · 2024★★★★

Evergreen Woods has a concerning pattern of recurring deficiencies across multiple surveys, with one complaint-triggered issue involving dietary accommodations for resident allergies. The facility repeatedly struggles with respiratory care (found deficient in three separate surveys), care planning and assessment, and safety systems. While all violations show correction dates, the frequency of repeat issues across quality of care, safety protocols, and administrative oversight suggests ongoing operational challenges that families should investigate further.
