What this means for your family

This facility's strong rehab therapy team makes it a solid choice for post-surgery recovery. However, families should be vigilant regarding call-bell response times and should personally monitor their loved one's daily hygiene and medication adherence, as these areas were frequently cited as inconsistent.

Questions for Your Tour

  • 1Given the facility's strong focus on post-surgery rehabilitation, what specific steps do you take to ensure consistent communication between the therapy team and the nursing staff regarding a resident's daily progress?
  • 2I noticed some feedback regarding the dining experience; could you walk me through how the kitchen team ensures meal temperatures are maintained and that dietary preferences or menu updates are accurately tracked for each resident?

  • 4I see that the facility actively responds to feedback online; how does management use that input to address concerns about nursing consistency and overall care coordination?

Key Review Excerpts

The only caring nurse in this facility is Jamie. Therapy staff are excellent and very caring. If you call for help because you have soiled yourself, be prepared to wait for hours.

Family member · 2025★★☆☆☆

The nursing staff, for most part took great care of my mother. As in most places there are a few employees that did not do their jobs well and management could do a better job with making sure all employees are treating every patient with kindness and compassion.

Long-term resident's family · 2025★★★★

I was nervous about having her there due to all the horror stories I had heard about rehab facilities, but knew she needed rehab to gain her strength. The staff was absolutely amazing.

This facility shows consistent deficiency patterns across multiple surveys, with recurring issues in resident rights, medication management, and care planning that persist from 2019 to 2024. One family filed a complaint resulting in a citation for medical records management. While all deficiencies show correction dates, the repeated nature of violations in core care areas suggests ongoing operational challenges that families should carefully consider.
