The Licensing Program Analyst observed that the facility was clean, well-stocked, and that all required safety equipment, including fire extinguishers and alarms, was fully operable. No deficiencies were issued during this inspection.

The facility underwent an unannounced annual required visit. The Licensing Program Analyst observed that the physical plant, outdoor area, and kitchen were clean and in good repair. All reviewed documentation, including staff and resident files, was found to be complete and compliant.

The facility underwent an unannounced annual inspection conducted by the LPA. The inspection covered general facility areas, resident records, and medication storage. No deficiencies were observed during this inspection.

The investigation substantiated allegations regarding unlawful eviction practices. The facility was cited for failing to provide the legally required written notice period for resident eviction and for general non-compliance with established eviction procedures. Corrective action is required, including staff training and submission of verification to the licensing authority.

The facility underwent an annual inspection focused on infection control. While general infection control measures appeared adequate, one deficiency was cited regarding the storage of dangerous items in the care of residents with dementia. The facility was advised of the citation and provided a Plan of Correction due date.

The inspection was conducted to ensure the facility is following the California Code of Regulations, Title 22, Division 6. Overall, the facility was found to be clean with no deficiencies observed per Title 22 Regulation. However, the administrator was advised to update the facility's COVID policy to reflect current departmental guidelines.
