The facility underwent an Annual 1-year required inspection on 12/09/2025. The physical plant, food storage, and basic safety measures were observed to be in order. However, four Type B deficiencies were cited, primarily related to missing staff health documentation (TB results), lack of recent emergency drill records, and failure to submit required administrative documents like insurance and LIC 500.

The facility underwent an Annual Continuation visit on December 21, 2023. The Licensing Program Analyst interviewed three residents and two staff members. No deficiencies were cited during this inspection.

The inspection was conducted on November 17, 2023, as a required Annual inspection. Several deficiencies were noted, including expired food, an unsecured medication room, and accessible cleaning supplies. The facility staff addressed these issues during the visit, and the LPA/LPM provided corrective guidance on securing these areas.

The unannounced annual inspection identified one deficiency related to infection control documentation. Specifically, the facility was cited for inconsistent daily COVID-19 screening logs for staff. Corrective action is required to ensure all staff are screened daily and documentation is maintained.

The unannounced Case Management inspection identified deficiencies related to pet policy documentation for a resident admitted with a cat. Specifically, the Admission Agreement lacked a pet policy, and the required Addendum for pet deposits and service charges was not completed for the resident in question.
