The facility failed to ensure necessary referral and follow-up care for a resident, specifically regarding a missed CT scan appointment. Although an imaging order existed, the manager failed to arrange transportation for the rescheduled October appointment due to personal family matters.

The facility failed to ensure that 4 out of 6 sampled staff members had documentation of a North Carolina Health Care Personnel Registry (HCPR) check completed prior to their hire dates. While subsequent checks performed during the survey showed no substantiated findings, the required pre-hire verification was missing from the personnel records of Staff A, Staff B, Staff D, and Staff F.

The facility failed to implement CDC and NC DHHS guidance regarding COVID-19 protections. Specifically, staff were not wearing personal protective equipment (PPE), the facility was not conducting required screenings for staff, residents, and visitors, and residents were not practicing six-foot social distancing during communal dining.

Interviews revealed that while the Administrator was aware of the suspension, a Supervisor was unaware and assisted with the admission.
