Resident A did not have a Health Care Appraisal (HCA) dated for 2024 or 2025; the last HCA was dated 09/2023.

Resident C’s window did not contain a screen.

Medication administration errors: Resident E missed Omeprazole and Vitamin D3 on multiple dates; Resident D missed Vitamin D3 on multiple dates; Resident C missed Probiotic Complex on multiple dates.

Resident C’s window sill was found to be damaged and in need of repair.
