Staff member B was suspended on 05/15/2025 following an audit and terminated on 06/02/2025 after receiving disqualifying background check results. A separate cover letter indicates deficiencies 64412 and 61958 were corrected as of 08/21/2025.
The provider failed to complete a background check upon hire for one staff member, resulting in a staff member with a disqualifying conviction having unsupervised access to clients.
