Specifically, while orders existed for weekly testing, a review of the glucometer and medication records showed a lack of recent or accurate documentation of these required tests.

The facility failed to ensure foods were protected from contamination by failing to properly store and label items. Observations revealed expired yogurt, undated cooked spaghetti, and undated opened cans of sauce. Additionally, various frozen meats in the refrigerator and deep freezer were found without required dates or labels.

The facility failed to prepare menus at least one week in advance with specified serving quantities. During the inspection, no menu was posted in the kitchen, and lunch service consisted of items like fried bologna sandwiches and honey buns without a documented nutritional plan.
