Key Review Excerpts

The staff are great and it appears there is little staff turnover. Several staff members have been there for over 20 years. When we visit, or participate in activities, we witness the genuine care, love, patience, and compassion the staff have with the residents.

Long-term resident's family · 2024★★★★★

The therapists, CNAs, medical staff and especially the administration support staff, had great care and communication throughout my father’s stay. My father is back home now due to outstanding efforts of Kelly, Social Services.

Rehab patient's family · 2025★★★★★

We've had a long association with Midlands. Our aunt and my mother resided there. Our uncle has been there for almost nine years. He feels that he has it made at Midlands. He has everything he needs.

4 complaint-triggered

Midlands Living Center has persistent issues across multiple care areas, with families filing four complaint-triggered deficiencies. The facility repeatedly struggles with infection control (cited four times across three years), care planning and assessment (five citations), and safety supervision. While all violations show correction dates, the recurring problems with infection prevention, care plan development, and accident prevention suggest ongoing quality challenges that families should investigate thoroughly during visits.
