Specifically, for Resident #4, the record failed to document the administration of PRN acetaminophen despite physical evidence of medication usage, and the record lacked necessary documentation for medication used for agitation in another resident.

The facility failed to ensure that physician orders for the application and removal of graduated compression socks were properly implemented and documented. Specifically, for one resident, there was no documentation of the application and removal of TED hose from January 18, 2022, through the end of the month.

The facility failed to implement and document physician orders for the application and removal of compression stockings for a resident with chronic venous insufficiency. Review of eMARs showed no documentation of the ordered TED hose application from January 18, 2022, through April 2022. An observation of the resident revealed she was not wearing the required compression stockings and exhibited leg swelling.

The facility failed to ensure that 11 of 11 small oxygen tanks were properly racked and free of hazards. Specifically, unracked oxygen tanks were observed stored in a cardboard box inside a resident's room rather than in the designated oxygen storage room.
