The facility failed to ensure that three sampled staff members (Staff A, B, and C) had documentation of a two-step tuberculosis (TB) skin test. The Administrator noted that while an audit was performed in March 2025, the lack of required TB testing documentation was missed.

The facility failed to ensure one medication aide completed the required six hours of annual continuing education related to medication administration. Personnel records showed only two units of education for 2019 and no relevant units for 2018.

The facility failed to ensure one sampled staff member received a two-step tuberculosis skin test upon employment. The Business Office Manager failed to follow up to ensure the required items in the employment package were completed.

The facility failed to complete a criminal background check for a staff member in accordance with state requirements.
