In the kitchen and back common area there are several cracks in the flooring that need to be repaired.

Direct care staff (Ashley Selley & Emma Johnson) were found to be disrespectful to a resident; specifically, one staff member used a sharp tone and another was heard saying 'Shut the fuck up' on a recorded voicemail.

Five direct care workers were missing current first aid and cardiopulmonary resuscitation (CPR) training; specifically, Jordan Wilson's CPR expired in July 2024, and others (Ashley Selley, Cassie Bourland, Tina Heckard, and Lomiaya Reeves) had not completed CPR since being hired.
