The facility failed to ensure that table service included non-disposable place settings. During breakfast and lunch observations, residents were served meals on disposable dinner plates with plastic forks and knives.

Specifically, three residents (#1, #2, and #3) with cognitive and/or physical impairments were identified who could not independently evacuate the facility, whereas the license was for ambulatory residents.

Specifically, for 13 of 19 night shifts between April 1 and April 19, 2016, the only staff member on duty (Staff C) lacked documented CPR certification.
